Name Change on the Illinois Supreme Court's Roll of Attorneys | State of Illinois Office of the Illinois Courts

The Illinois Supreme Court's Roll of Attorneys ("Roll") is a list of all attorneys admitted to practice law in Illinois. The Roll includes the names of lawyers even if they have not registered with the Attorney Registration and Disciplinary Commission ("ARDC"). The Roll is maintained by the Clerk of the Illinois Supreme Court. A lawyer's name on the Roll will reflect the name under which the lawyer was admitted to the practice of law in Illinois or any subsequent name change allowed by the Court.

 In order to change your name on the Illinois Supreme Court's Roll of Attorneys, you must file a motion with the Clerk of the Supreme Court requesting to change your name on its Roll. The motion may be in the form of a formal letter. The motion/letter should include the name under which you are currently listed on the Roll (usually, the name under which you were originally admitted) and your new legal name. Your address and phone number (either home or business) should also appear on the motion/letter.

 You must provide documentation evidencing your name change. Examples: If your name change is the result of a marriage, please attach a certified copy of your marriage certificate. If your name change is the result of a dissolution of marriage, please attach a copy of the dissolution judgment attesting to your name change, or a copy of any separate change of name order entered in the dissolution proceedings. If your name change is the result of a court-ordered change of name, please attach a copy of the order and explain in the letter of request why you have changed your name. While the copy of the designated document need not be certified, it must be a copy of a certified copy.

 You need not serve the ARDC with a copy of the motion. If the Court allows your motion, the Clerk of the Supreme Court will notify you and the ARDC. The ARDC will then change your name on its Master Roll of registered attorneys and will send you a new identification card at your registered business address.
